Q: What should I expect at my first appointment?

Your first appointment will last about an hour. Your physical therapist will take a detailed history and perform a thorough examination. If you are being treated for a pelvic health condition, your therapist may perform a brief internal examination with your consent. We will then work together to establish a treatment plan. Q: How much does Physical Therapy cost?

In-Network Services: We are continuously adding new insurance companies. We will verify your insurance benefits before your first appointment for physical therapy. Depending on your insurance, you may have no out of pocket cost. Co-pays and deductibles are due at time of service. We also offer wellness services that are not covered by insurance and are cash pay. Out-of-Network Services or Cash Pay: $140 for the first appointment and $120 per followup. We can provide you with a superbill for you to submit to your insurance company if we are out-of-network with your insurance.

Q: Do I need a physician referral?

Many of our patients receive a referral from their physician or health care provider for physical therapy services. PTs do not need a physician referral to treat patients in Arkansas, however, your insurance company may require a referral to cover your treatment. Q: How long are treatment sessions?

All treatment sessions are one hour long and you'll spend the entire time with a licensed doctor of physical therapy. We don't pass you off to assistants or technicians like many other clinics.

Q: What should I bring to my appointment?

Please bring your insurance card, a valid photo ID, a list of current medications, and any relevant medical records or imaging studies. Wear comfortable clothing that allows easy access to the area being treated.

Q: What insurance do you accept?

Q: How often will I need to come in?

Treatment frequency varies based on your condition and goals. Typically, patients are seen 1-2 times per week initially. Your therapist will work with you to develop a schedule that fits your needs and promotes optimal recovery.

Q: What is your cancellation policy?

We require 24 hours notice for appointment cancellations. Late cancellations or no-shows may be subject to a fee. We understand that emergencies happen, so please call us as soon as possible if you need to reschedule.

Q: What are your office hours and location?

Our Bentonville office is located at 210 N Walton Blvd, Suite 26, Bentonville, AR 72712. We are open Monday through Friday from 8:00 AM to 5:00 PM. We are closed on weekends and major holidays. Our Rogers clinic is located on the 2nd floor within the Great Southern Bank building at 2522 S Pinnacle Hills Pkwy Suite 200A, Rogers, AR 72758.
